儿童外伤非常重要,是儿童死亡最常见的原因,幸运的是,绝大多数外伤为轻型外伤。其中以头部外伤最为常见,由于儿童头部占到身体的 1/4,具有创伤高风险,但尚未闭合的囟门可对颅内血肿起缓冲作用。故绝大多数脑外伤是轻型的,一般不需要干预。

脑震荡,由创伤性生物力学因素引起的一个复杂的病生理过程,通常由直接打击或冲击力导致快速发病的短暂性损伤,无结构异常。如果小大人有更符合标准的头骨,允许更大的损伤,更大的炎症反应等,那么小大人与儿童相似。躯体、认知、情感或行为改变,健忘,头晕等都是脑震荡的症状。而其治疗主要是认知休息。

反复脑震荡会导致严重的再次冲击综合征,且儿童发病率较高。对此一般是对症药物治疗,但最好避免再次损伤。此外还会导致慢性创伤性脑病,同时提高抑郁症及阿尔兹海默症的发病率。

至于儿童伤口处理,不仅具有技术上和情感上的挑战,还要分散其注意力,也可进行束缚治疗,而局麻药的应用,可以让处理更轻松,并可使用可吸收缝线。缝合时需使创缘外翻,不能太紧、太多、太宽。不同部位,拆线时间不同。同时注意是否需要使用破伤风疫苗。最后还需对父母进行伤口护理宣教。
