儿童肥胖在过去二十年内增长了 3 倍,已经成为全球严重的健康问题。而屏幕时间是造成肥胖的最大因素。因为儿童坐在银屏前时间越长,摄入垃圾食品的可能性越大。这些垃圾食品,营养低、能量高,还会影响机体代谢,最终造成儿童肥胖。而肥胖儿童大部分成年后仍伴肥胖。

在丁香诊所「全科医生强化培训班」中,来自美国南加州大学医院的儿童急诊副教授 Emily Rose 与大家一起讨论《儿童肥胖》。

儿童肥胖常有多种并发症。常见的并发症有:心血管疾病、糖尿病、哮喘、胆结石、肌肉骨骼系统疾病、多囊卵巢综合征、脂肪肝、假脑瘤症等。其中,心血管疾病风险在肥胖儿童中是普通人的 2 倍。由于肥胖儿童易发生胰岛素抵抗,其患 2 型糖尿病概率大大增加。

但儿童糖尿病更易被忽视,缺乏常规治疗,使其比一般糖尿病进展更严重。随着儿童的增重,骨骼运动系统负重增加。肥胖儿童,其骨骼负重更多,容易造成其骨骼畸形或影响其生长发育。

此外,肥胖还可能并发假脑瘤症,尤其是女性患者。该类患者常表现出头痛、视力改变以及搏动性耳鸣。而该病目前无明确治疗方法,只能对症治疗。药物上可用碳酸酐酶抑制剂减少脑脊液产生。治疗上,可采取腰穿排放脑脊液。严重者需要手术治疗。同时,低钠饮食以及减重也是有效治疗措施。

肥胖儿童可通过健康饮食、不久坐、勤锻炼三个方法来达到减重目的。对于儿童来说,肥胖不仅仅有更多疾病并发症,更是给他们造成社会心理问题。医生如果能更早地进行干预、治疗肥胖儿童,其治疗效果更好。
